This policy explains how the MomDoc website (momdoc.com and its subdomains) collects, uses, and protects information when you visit our site. This is separate from our HIPAA Notice of Privacy Practices, which governs how we handle your medical information.

Google Analytics

Sentry Error Tracking
We use Sentry to detect and fix technical errors on our site. When an error occurs, Sentry collects the error message, the page URL, your browser type, and a stack trace of the error. Sentry does not collect your name, email, or other personal information.

Information You Provide
When you voluntarily submit information through our website forms, we collect what you provide:
Appointment Requests
Name, phone number, email address, preferred location, and reason for visit. This information is transmitted to our scheduling team to arrange your appointment.
Event RSVPs
Name, email, number of guests, and any dietary or accessibility notes. This information is stored securely and used solely to manage event attendance.
Contact Forms
Name, email, phone number, and your message. This information is used to respond to your inquiry.
We do not sell, rent, or share your form submission data with third parties for marketing purposes.
Service Worker
Our site uses a service worker to cache static assets (images, stylesheets, scripts) on your device for faster loading on repeat visits. The service worker does not cache or transmit personal data. You can disable service workers in your browser settings.
Third-Party Services
Our site loads resources from the following third-party services:
- Google Maps for office location maps
- YouTube for embedded practice videos
- Google Fonts for web typography
- Jotform for appointment request submissions. Jotform is a HIPAA-covered form provider; submissions post directly from your browser to Jotform's infrastructure under an executed Business Associate Agreement with MomDoc.
Each of these services has its own privacy policy governing data collection when their content loads on our pages.
Data Security
We use HTTPS encryption for all data transmitted between your browser and our servers. Our Content Security Policy restricts which external resources can load on our pages. Form submissions are validated and sanitized to prevent injection attacks. API endpoints are rate-limited to prevent abuse.
Children's Privacy
Our website is not directed at children under 13. We do not knowingly collect personal information from children. If you believe a child has submitted personal information through our forms, please contact us so we can delete it.
Your Choices
- Analytics opt-out (this site): Use the analytics preferences control near the bottom of this page to disable Google Analytics for this browser. The setting is stored on your device and persists until you clear site data.
- Analytics opt-out (all sites): Install the Google Analytics Opt-out Browser Add-on to prevent Google Analytics from collecting data across every website you visit.

- Form data: Contact us to request deletion of any information you submitted through our forms.
