Payment Options

At MomDoc, all women are seen. We offer multiple ways to pay for your care, and our billing team is here to help you find the option that works best for your situation.

HSA & FSA Cards

Health Savings Account and Flexible Spending Account cards are accepted for all eligible medical services, copays, and deductibles.

Credit & Debit Cards

We accept Visa, Mastercard, American Express, and Discover. Pay in person at any MomDoc location or by phone with our billing team.

Cash & Check

Accepted at all MomDoc locations for copays, balances, and self-pay visits.

Payment Plans

Cannot pay in full? We offer flexible payment plans for larger balances. Call our billing team to set up monthly installments that fit your budget.

Self-Pay Pricing

If you do not have insurance or prefer to pay out of pocket, MomDoc offers transparent self-pay pricing. Under federal law (the No Surprises Act), you have the right to receive a Good Faith Estimate of expected charges before scheduling non-emergency services.

How it works:

  1. Call 480-821-3601 and let us know you are a self-pay patient
  2. Our team will provide a written estimate for the services you need
  3. You pay at the time of service or arrange a payment plan in advance

Self-pay rates are often significantly lower than billed insurance rates because they eliminate the administrative overhead of claims processing. Ask our billing team for current pricing on your specific service.


Global Maternity Package

For self-pay maternity patients, we offer a Global Maternity Package that bundles your entire prenatal care, delivery, and postpartum visit into one predictable fee. The package includes:

  • All routine prenatal office visits
  • Standard prenatal lab work
  • Your delivery (vaginal or cesarean)
  • One postpartum follow-up visit

What is not included: Hospital facility charges, anesthesia, specialized imaging beyond standard prenatal ultrasounds, and any complications requiring additional procedures are billed separately by the hospital or other providers.

Financial Hardship Assistance

MomDoc was founded on the principle that no one will be denied access to services due to inability to pay. We offer several programs to help patients who are experiencing financial difficulty:

Sliding Fee Scale

Based on Federal Poverty Guidelines, your family size, and your household income, you may qualify for reduced fees. To apply:

  • Provide proof of income (pay stubs, tax return, or a signed statement)
  • Complete a brief application with our billing team
  • Fees are adjusted at the time of service

AHCCCS Enrollment Support

If you are uninsured, you may qualify for AHCCCS (Arizona Medicaid) coverage. MomDoc has an on-site Health-e-Arizona Assistor who will sit with you, complete your application, submit your documents electronically, and track it through approval. There is no cost for this service.

Pregnant women who qualify pay no monthly premiums and no copays. Coverage continues for 12 full months after delivery. Income limits for a single person are $2,075/month; for a family of four, $4,290/month.

Federal Emergency Services (FES)

MomDoc proudly accepts FES for maternity care. While FES primarily covers your hospital delivery, we provide comprehensive prenatal care for FES patients because every baby deserves a healthy start.


Payment Plans

For balances over a certain threshold, we offer interest-free monthly payment plans. Our billing team will work with you to determine a monthly amount that fits your budget.

To set up a payment plan:

  • Call 480-821-3601 and ask for billing
  • Plans are typically 3 to 12 months depending on the balance
  • Payments can be made by credit card, debit card, or bank transfer
  • You can set up automatic monthly payments for convenience

Understanding Your Bill

Healthcare billing can be confusing. Here are a few things to keep in mind:

You may receive multiple bills. Lab work (Sonora Quest, LabCorp), hospital facility charges, and anesthesia are billed by separate entities, not by MomDoc. If you deliver at a hospital, expect a separate bill from the hospital and from the anesthesiologist.

Check your Explanation of Benefits (EOB). Your insurance company sends an EOB after processing a claim. An EOB is not a bill. It shows what was billed, what insurance paid, and what you owe. Wait for your actual statement from MomDoc before paying.
