How the online request works
Most patients use ChartRequest, a secure third-party service. You leave momdoc.com only to complete the request. We do not collect medical record details on this page.
- Open ChartRequest (button above or the link below).
- Create an account or sign in.
- Enter patient details and the dates of service you need.
- E-sign the authorization and review any estimated charges ChartRequest shows before you finish.
- Track status in ChartRequest until records are ready to download or deliver.
