These specialty services may or may not be covered by your insurance. To manage the costs of these services, our reimbursement schedule is formatted to accept insurance for Provider time and cash pay for specialty services.
To facilitate payment of these services, we have established the following policy:
- Co-payments will be collected at appointment check in.
- We will bill your insurance for office visits and eligible services.
- If you are receiving a specialty service, payment in full will be required at check in.
- Coinsurances and deductibles remain the responsibility of the patient.
- Any service rendered not covered by your insurance policy will remain your responsibility.
