As a courtesy to our patients with limited or no insurance coverage, we offer reduced rates for certain services. The reduced rate must be paid in full and in advance, otherwise, standard rates will apply. Reduced rates do not include additional services or procedures, laboratory services, or any other studies performed. Reduced rates are subject to change without notice.

The listed price below will be charged at the time of your appointment. The total price may be more, depending on tests or procedures that are ordered or performed, and the complexity of your visit. At the end of your visit we will assess the charges. The possible price range is noted in brackets.

To facilitate payment of these services, we have established the following policy: 

  1. Co-payments will be collected at appointment check-in
  2. We will bill your insurance for office visits and eligible services
  3. If you are receiving a specialty service, payment in full will be required at check-in
  4. Coinsurance and deductibles remain the responsibility of the patient
  5. Any service rendered not covered by your insurance policy will remain your responsibility

 

This policy will allow us to offer our patients competitive pricing while offering quality management of care.
